磁共振ZTE、T2 mapping序列扫描对膝关节软骨损伤的诊断价值

摘要:

目的 探讨磁共振零回波时间(ZTE)和T2 mapping序列扫描对膝关节软骨损伤的诊断价值。方法 选取600例疑似膝关节软骨损伤患者,均行磁共振ZTE、T2 mapping序列扫描。以关节镜检查结果为诊断金标准,比较磁共振不同序列扫描对膝关节软骨损伤的诊断效能。结果 600例疑似膝关节软骨损伤患者,关节镜检查阳性492例、阴性108例;磁共振ZTE序列扫描阳性447例、阴性153例;磁共振T2 mapping序列扫描阳性479例、阴性121例。磁共振T2 mapping序列扫描诊断膝关节软骨损伤的灵敏度、准确率和阴性预测值均高于磁共振ZTE序列扫描,漏诊率低于磁共振ZTE序列扫描(P均<0.05);而磁共振两种序列扫描诊断膝关节软骨损伤的特异度、误诊率和阳性预测值比较差异均无统计学意义(P均>0.05)。结论 相较于磁共振ZTE序列扫描,磁共振T2 mapping序列扫描对膝关节软骨损伤的诊断价值较高。

Abstract:

Objective To explore the diagnostic value of zero echo time(ZTE) and T2 mapping sequences for knee joint cartilage injury. Methods A total of 600 patients with suspected knee joint cartilage injury were selected, and all of them underwent ZTE and T2 mapping sequence scans. Using arthroscopy as the gold standard for diagnosis, the diagnostic efficacy of different MR sequences for knee joint cartilage injury was compared. Results Among the 600 patients with suspected knee joint cartilage injury, 492 cases were positive and 108 cases were negative according to arthroscopy. 447 cases were positive and 153 cases were negative according to MR ZTE sequence scans. 479 cases were positive and 121 cases were negative according to MR T2 mapping sequence scans. The sensitivity, accuracy, and negative predictive value of MR T2 mapping sequence scans for diagnosing knee joint cartilage injury were all higher than those of ZTE sequence scans, and the missed diagnosis rate was lower than that of ZTE sequence scans(all P < 0.05). However, there were no statistically significant differences in specificity, misdiagnosis rate, and positive predictive value among different MR scanning sequences for diagnosing knee joint cartilage injury(all P > 0.05). Conclusion Among the ZTE and T2 mapping sequences, the T2 mapping sequence has the highest diagnostic value for knee joint cartilage injury.
