速度向量成像技术评估子痫前期胎儿右心室心肌节段功能的价值

摘要:

目的 应用速度向量成像技术(VVI)评估子痫前期胎儿右心室心肌节段功能的变化。方法 选取20例孕妇子痫前期(preeclampsia,PE)的胎儿(实验组)及20例正常胎儿(对照组)的标准四腔心切面动态声像图,比较两组胎儿右心室游离壁和室间隔右心室面各节段收缩期峰值速度(Vs)、舒张期峰值速度(Vd)、峰值应变(S)、收缩期峰值应变率(SRs)及舒张期峰值应变率(SRd)。结果 两组心肌右心室长轴上基底段至心尖段Vs、Vd均有显著的梯度下降趋势(P<0.05);基底段、中间段和心尖段S、SRs、SRd,其数值差异无统计学意义(P> 0.05)。实验组Vs较对照组差异无统计学意义(P> 0.05),Vd、S、SRs、SRd较对照组减低,差异有统计学意义(P <0.05)。结论 子痫前期的胎儿心肌长轴上右心室已出现节段性功能降低; VVI可以客观、准确地反映胎儿心脏功能的变化。

Abstract:

Objective The primary aim of this study was to evaluate the changes of fetal right ventricular longitudinal myocardial function of using velocity vector imaging( VVI) in fetuses with preeclampsia. Methods Digital dynamic four-chamber views of 20 fetuses with preeclampsia and 20 normal fetuses were collected and analyzed by velocity vector imaging technique. The regional tissue systolic peak velocity( Vs),diastolic peak velocity( Vd),peak strain( S),systolic peak strain rate( SRs) and diastolic peak strain rate( SRd) of right interventricular septum and right free wall were compared between the two groups. Results Observing the data from the basal segment to the apical segment of the fetuses in the two groups,we found that the velocity of the right ventricular septum and the right ventricular free wall had a significant gradient decline trend( P < 0. 05),but S,SRs and SRd were not found to have significant differences among all segments in the two groups( P > 0. 05). Compared with the control group,Vs in PE had no statistical significance( P > 0. 05),Vd,S,SRs and SRd were lower than those in the control group( P < 0. 05). Conclusion The longitudinal myocardial segmental function of the right ventricular in fetuses with preeclampsia has been reduced. The alterations in fetal cardiac function can be mirrored by the VVI precisely and objectively.
